On this page, you can generate a full insulin sliding scale for people on multiple daily injections, based on carb ratio (which must be between 4–60 grams), correction/sensitivity factor (ISF, which must be between 1–20 mmol/L or 18–360 mg/dL), and target blood glucose (the default is 6 mmol/L or 100 mg/dL). Insulin doses are rounded off (using a cut-off of 80%, not 50%) to the nearest half-unit. Calculations do not account for active insulin/insulin-on-board, nor for activity! You can export the resulting table to a CSV file, which can be opened in any spreadsheet program (Microsoft Excel, Google Sheets, Mac OS Numbers, etc.).
